Instructions:

1. All five (5) forms must be completely filled out in order for Puritan Products to set up your company as a customer.
   • Exceptions:
     • Sales Tax Exemption form—only needs to be filled out if your company is located in the United States.
     • If your company is located outside of North America, there is no need to fill out a Sales Tax Exemption form or a Credit Application—payment in advance will be required.
2. Please type your company’s information into the forms, click on the print button at the bottom of each form, and fax the completed forms to 610-866-4270. The forms can not be saved as documents on your computer unless you have Adobe Reader 8 installed on your computer.
3. If you need help with the forms, please call Customer Service at 610-866-4225, option 2, during normal business hours (Monday through Friday 8 am to 5 pm EST/DST).
